I/We are applying for membership with COMTRUST Federal Credit Union. I/We authorize
COMTRUST Federal Credit Union to access my/our account and/or credit history. I/We also
authorize COMTRUST Federal Credit Union to obtain reports from third parties – including

that I/we request. I/We further authorize COMTRUST Federal Credit Union to use these
